Do you take insurance?
We do not accept insurance. We are an out-of-network provider with most PPO Plans. This means that you are responsible for paying for the evaluation. We can provide you with a “superbill” to submit to your insurance company for reimbursement. Insurance plan coverage can vary greatly, so please check with your provider via phone or online for specific coverage and rates of reimbursement.
Here are some helpful questions to ask your insurance company:
- Do I have mental health coverage?
- What percentage of my bill will be covered for services provided by an out-of-network provider?
- Is approval required from my primary care physician?
- What is my co-payment for an outpatient office visit?
- What percentage of services is covered after my co-payment?
- Do I have co-insurance on the plan? If so, what is the percentage?
- Do I have a deductible and has it been met?
Ask your insurance company about coverage for the following CPT Codes:
- 96130 and 96131- Psychological testing and evaluation by psychologist
- 96132 and 96133 – Neuropsychological testing and evaluation by psychologist
- 96136 and 96137 – Psychological or neuropsychological test administration and scoring by psychologist
You should also verify if “educational testing” is a covered. They may ask for a specific diagnosis code in order to answer this question. You could give them the following codes to check:
- 314.01 – ADHD (ICD code is F90.2)
- 315.00 – Reading Disorder (ICD code F81.0)
- R48.0 – Dyslexia
Forensic evaluations are not covered by insurance.
How can I get reimbursed from my insurance?
We can provide you with a “superbill” that outlines the services that were provided. You will then have to submit the forms to your insurance for reimbursement.
We have partnered with Reimbursify to help you with filing claims.
Reimbursify is a mobile app that allows you to file claims from any smartphone. They charge $1.99 to process an individual claim. You can use the code REIMBURSIFVE to get your first 5 claims for free. You can visit reimbursify.com or contact hello@reimbursify.com for more information.
Insurance does not cover forensic evaluations.
How much does an evaluation cost and what is included in the cost?
Our fee for a comprehensive neuropsychological evaluation is $250 per hour. Evaluations typically include the following:
- 1-2 hour interview.
- 2-6 hours of testing. This can vary greatly depending on the issue and referral questions that need to be addressed.
- 1-2 hours of scoring.
- 3-6 hours of report writing.
- 1-hour feedback session.
- The total evaluation time can range from 8 to 17 hours. These guidelines can vary depending on the referral questions that need to be addressed.
A 50% deposit (based on the estimated cost of the evaluation) is required at the first session. The remaining balance is due after the final feedback session.
Forensic evaluations have a different fee schedule. The cost can vary depending on the type of evaluation. Please contact us for a free consultation to discuss your case.
Why do I need a neuropsychological evaluation?
A brain scan can identify areas that have been injured but it does not provide information on how that injury is impacting your day-to-day functioning. A neuropsychological evaluation will help identify how your brain is working and how it is impacting your cognitive abilities or mood.
Clients commonly ask us the following questions:
- When can I go back to work again?
- Can I start driving?
- My memory is getting worse, is there something wrong with me?
- I have a hard time with attention and concentration, do I have ADHD?
- What kind of accommodations do I need for school?
- I’m depressed and anxious, how is it impacting my thinking or memory?
The evaluation can help answer these questions and provide treatment recommendations to improve your quality of life.
What will neuropsychological tests measure?
Neuropsychological tests examine the following areas of cognitive and emotional functioning:
- General Intelligence
- Academic Abilities
- Problem Solving
- Thinking and Reasoning
- Attention and Concentration
- Learning and Memory
- Language Comprehension and Communication
- Visual and Spatial Perception
- Motor and sensory skills
The evaluation will focus on specific areas of functioning that are impaired due to your illness or injury. We will also assess for psychological conditions such as depression or anxiety to determine how they are impacting your functioning.
How will a neuropsychological evaluation help me?
A neuropsychological evaluation will provide you with:
- A better understanding of your cognitive strengths and weaknesses.
- Information for your doctors so they can develop a treatment plan to address the problems you are having.
- Recommendations for your day-to-day life in order to improve your quality of life.
- Recommendations for accommodations in school or work to help you perform at your true potential.
What will I have to do on the day of my evaluation?
We will meet with you and anyone that you would like to bring to complete an interview that covers your background and history. The interview will help us have a better understanding of your current symptoms. We are also able to complete your interview before your testing session via video chat. Your family can leave while you are tested.
After the interview, we will administer various tests in order to answer the specific questions or concerns that you or your doctor have. Some of the tests are paper and pencil, some are administered orally, and others are administered on the computer. We will explain and demonstrate each test before we administer them and answer any questions you might have.
How can I prepare for my evaluation?
- We will e-mail you questionnaires to complete and bring them with you to your appointment.
- Make sure you get a good night’s sleep the night before.
- Make sure you eat something before you arrive so that you are comfortable until the lunch break.
- If you wear hearing aids or eyeglasses, bring them with you.
- Take your medications as you normally do, unless your doctor has told you otherwise.
- Feel free to bring a snack or beverage. We will take a break for lunch if needed.
- Bring any medical records that are related to your current illness or injury.
- If you are in school, bring your current academic records.
- Please do not consume any alcohol or drugs 24 hours prior to the evaluation.
Do I need to study before I take these tests?
The tests you will be administered do not require any preparation or studying. The tests try to get a sense of your current abilities, so there is no need to learn any new material or practice any skills. The tests are not pass or fail. The tests help identify your strengths and weaknesses in different areas of psychological and neuropsychological functioning.
Who will get the results of my evaluation?
Forensic reports are not confidential and will be provided to your attorney and/or court.
Do I have to come back to get my results?
Feedback sessions are not provided for forensic evaluations.
What forms of payment do you accept?
We accept cash, cashier’s check, debit cards, and all major credit cards. We do not accept personal checks. Credit and debit cards are subject to a 3% transaction fee.
